This is for the two things that oblige somebody to act: work used without permission, and a mod that does something to the machine it runs on. Anything else - it is broken, it is bad, it is not what it says - belongs in a comment on the mod, where its author can answer.

What is wrong
What it did, or what it contains and whose it is. Be specific - somebody has to act on this, and a report nobody can check is one nobody can act on.

You do not need an account. If you are signed in it is filed as you, which helps whoever reads it come back to you; either way nothing about your machine is recorded.
